Completing a general application form
for entry from September 2008
 
Guidance Notes on this page relate to the form found at the bottom of this page available to download, you are also advised to read the frequently asked questions in the previous section for guidance. The General Application Form is available to download at the bottom of this page.
 
Section 1 – Pupil Details
 
Please make sure you fill in this section clearly and accurately, ensuring any unusual spellings are clearly printed. Use the address where you normally live with your child and, if you are new to Leicester, you must tell us if this is only temporary.
 
Section 2 – School Preferences
 
In this section you are requested to make your school preferences.
 
If you are basing your application on religious grounds, you need to tick the box indicated.  You will also need to provide written confirmation from your local priest or religious leader supporting your application.
 
It may be helpful, before completing this section, to talk to Admissions staff about which schools have spaces in your child’s year group. If the school is full then your application will be refused, but you will be given the opportunity to apply for a place on the oversubscription list (except for Year 11). The appropriate form will need to be completed.
 
In the meantime, if your child is not in school, you must accept an alternative school. This is because the oversubscription list relies on another child leaving your preferred school which may take some time. Therefore, by accepting the alternative offer, you are ensuring that your child continues to receive an education.
 
Section 3 – Your Child’s Educational History
 
To help your child start school as soon as possible, we need to know about your child’s current or recent past educational history. Missing out information may delay the starting date for your child.
 
Section 4 – Background & Language Ability (Optional)
 
This section is to enable the school to process and assess your child as soon as possible. We, therefore, ask that you complete this section.
 
We need information to help us to prioritise your application and to inform the relevant school so that they can assist newcomers to Leicester to settle in quickly.
 
Section 5 - Household Information
 
We need to know if you are caring for a child placed with you by a LA, i.e. are you a foster carer. If, ‘yes’ you must name the LA.
We also need to know if the child is on the 'List of Children Subject to Child Protection Plans' and, if so, please name and provide the contact details of your Social Worker.
 
Equally important is for you to indicate what your relationship is to the child. Normally, the birth parent with whom the child lives should apply, but for a child not living with either of their parents, another adult who has legal responsibility for the child may apply.
 
For the purpose of school admission, we need to know if the child has a brother or sister attending any of your preferred schools. This must be another child living at the same address. This includes stepbrothers or sisters, foster brothers or sisters and half-brothers or sisters. The School Admissions Service may ask for documentary evidence, such as your child’s benefit letter as proof of residence. Please note, a cousin does not count as a brother or sister.
 
Section 6 – Declaration
 
Once you have completed the application form, you must sign the Declaration to confirm that all the information you have provided is accurate. If we later discover that you have given false information, the place may be withdrawn. By signing here, you also confirm that your details will be shared with other Council departments and public bodies to prevent and detect fraud.
