Referrals or requests for advice to LAST about individuals or groups of pupils should be made by the SENCO to the school's designated teacher at the joint planning meeting. If the school has a concern about the progress of a pupil the following steps should be taken:
- Pupils should already have been reviewed at School Action.
- A LAST pupil referral form needs to be completed with all requested information completed and copy of IEP attached, plus Pupil in Context Profile where appropriate.
- Joint planning meeting should be the forum to match the needs of pupils to the appropriate service. In some cases joint working between LAST and other teams or services may be appropriate.
