Carers Assessment - Good Practice

The development of the personalisation agenda provides practictioners with opportunities to move away from gatekeeping towards a new role that is focused on outcomes.

Undertaking a carer’s assessment will require:

- An ability to empower the carer to self-assess.

- Enabling the carer to identify the outcomes that will meet their needs.

- Brokerage that will empower the carer to assemble the right ingredients for their support plans.

- Listening and counselling skills.

- An ability to enable the family to resolve conflict – the promotion of a whole family approach.

- The ability to identify the gaps in community support.

- Creativity.

- confidently undertake carer’s assessements that will meet the requirements of the personalisation agenda.

- have a very clear understanding of the legal framework and how to use it to maximise support for carers.

- fully understand the implication of the personalisation agenda for carers

- develop the skills that enable carers to make choices, to identify their priorities for the future and to assess the impact that caring will have on their lives.
