Carers Assessment - Good Practice

The development of the personalisation agenda provides practictioners with opportunities to move away from gatekeeping towards a new role that is focused on outcomes.

 

Undertaking a carer’s assessment will require:

 - An ability to empower the carer to self-assess.

- Enabling the carer to identify the outcomes that will meet their needs.

- Brokerage that will empower the carer to assemble the right ingredients for their support plans.

- Listening and counselling skills.

- An ability to enable the family to resolve conflict – the promotion of a whole family approach.

- The ability to identify the gaps in community support.

- Creativity.

    - confidently undertake carer’s assessements that will meet the requirements of the personalisation agenda.

have a very clear understanding of the legal framework and how to use it to maximise support for carers.

fully understand the implication of the personalisation agenda for carers

develop the skills that enable carers to make choices, to identify their priorities for the future and to assess the impact that caring will have on their lives.
