Social Care Records are the information we hold about people who receive services from us. This information is recorded on paper and/or held on computer.

 

It is essential in helping us to manage, plan and improve services. In some cases it is a legal requirement that we hold this information.

If you want to see your records you must make your request in writing to:

What we record

When contact is made with us, we record basic information such as.

 

  • Your name, address details, your date of birth, etc.
  • We keep a record of any services that we currently provide or have previously provided.
  • We keep details of relevant services provided to you by other organisations.
  • We keep information about your personal and family circumstances, as well as information given to us by people such as your doctor, may be recorded if relevant.
  • Our records include information on staff or agencies involved in the care of others, and may include details of individuals who come into contact with vulnerable children and adults.

Keeping records is an important part of our everyday work, and is a responsibility that we take very seriously.

What are your rights?

You have the right to see records that we hold about you.

 

  • You can also arrange for someone else to access your records on your behalf, although we need you to give permission for this in writing.
  • If you are unable to make a written application and do not have a friend or relative who can help, you can contact us and we will assist you to find help to do this.
  • If you have a social worker they will be able to explain what to do.
  • If you attend a day centre or residential establishment, a member of staff there will be able to advise you.

 

We have a legal duty, under the Data Protection Act 1998, to allow you to see information held on record, normally within 40 days of receiving your written request.

 

The Data Protection Act 1998 is the key legislation covering all aspects of information processing.

 

Personal information is also held subject to the Freedom of Information Act 2000, the Human Rights Act 1998 and common law.

 

We are very careful to ensure that all personal information that we hold is up-to-date, accurate and secure.
