This Statement of Purpose describes:

 

 

Learning Disabilities Provider Services include the:

 

 

This Statement of Purpose is available to anyone with an interest in Learning Disabilities Provider Services: Service users, Carers, Staff and any other interested persons.

 

Aims and Objectives 

The Learning Disability Service is committed to helping and supporting people with learning disabilities living in the City of Leicester.

 

This services aims to help and support people with a learning disability by:

 

  • Promoting and improving Independence

  • Increasing knowledge of Rights

  • Increasing opportunities for Choice

  • Promote Inclusion into their local community

 

The objective of this service is to:

 

  • Gather information about what support people needs, from their assessment and their Person Centred Plan and delivering services to meet these needs.

 

  • Provide support to take part in different activities in the community.

 

  • Support people with learning disabilities to be as independent as possible.

 

  • Increase the amount of choice and control people have in their lives.

 

  • Support people with learning disabilities to be included and feel safe in their local communities.

 

  • Provide health and social care services that specialise in working with people with learning disabilities. These include services for people with mental health problems, services for people who have difficulty communicating, and services for people with physical disabilities.

 

  • Use the quality assurance framework to measure the quality of outcomes and further develop our services.

 

The main driver to improve services is the white paper – Valuing People which sets out the national strategic direction for learning disabilities and reflects the objectives set out in the white paper ‘Our Health, Our Care, Our Say’ which sets out the new direction for community services for the Primary Care Trust, the NHS and Social Care.

 

Philosophy of Care

We aim to provide a ‘person centred’ approach to care. Person Centred means doing things in a way that the person wants, and which helps them to be part of their community. If someone is in the centre of something - they are the most important person.

 

Who the Service is for

 Service Users with learning disabilities who have been referred to the service by a care manager following an assessment according to FACS (Fairer Access to Care Services) Guidance.

 

Support for Carers

We will support carers in their role and enable them to be involved in the planning of services by:

 

  • Improving service and developing new services.

  • Giving service users and carers the chance to have a break from each other.

  • Liaising regularly with them.

 

How Cultural and Social needs are meet

We aim to meet cultural and social needs by:

 

  • Training our staff to increase awareness of cultural and social needs

  • Employing a diverse staff group

  • Providing activities to meet people with learning disabilities cultural and social needs

  • Planning future services to meet the changing cultural and social needs of people with Learning disabilities.
