Intensive Support Services

Intensive Support Services provide appropriate person centred services for adults with learning disabilities that have associated complex needs as identified in their Community Care Assessment and Care Plan.

 

Objectives

  • To receive referrals and plan appropriate services through the care planning process in accordance with FACS criteria.

     
  • Respond swiftly and effectively to referrals.

 

  • To provide a high quality, flexible, person centred service, which is monitored, reviewed and evaluated

     
  • To work effectively with all partners and agencies – at the centre of this will be the service user, their carers and advocates. 

     
  • To provide an appropriate range of social, sensory, educational, cultural and recreational activity that reflects the service user’s needs and choices.

     
  • To provide the appropriate resources, support and supervision to enable each services user to participate in each activity in a meaningful way and which will achieve positive outcomes.

     
  • To ensure that appropriate evidence based mechanisms are in place and used to evaluate individual service outcomes.

     
  • To provide a safe, comfortable and stimulating environment.

     
  • To ensure compliance with ‘Safe Guarding Vulnerable Adults’ multi- agency policy and procedure.

 

  • To provide a service that is reflective of the cultural diversity within Leicester.

 

  • To work with partners to promote good health and wellbeing.

 

  • To achieve a ‘total communication approach’, in which meaning is conveyed by a variety of different methods in addition to speech. This will maximise the potential for the service user to understand and will result in the services user being empowered to express their views. To support this process individual communication passport will be formulated.

 

  • To support service users with their person centred plans.

 

  • To support service users with intimate care tasks in an appropriate way that is respectful to the service user’s privacy and dignity and promotes their independence.

 

  • To raise the profile and presence of adults with a learning disability in the wider community.

 

  • To promote awareness and positively influence the perception that the public hold of adults with a learning disability.

 

  • To respond swiftly and constructively to any concern or complaint expressed.

 

  • To provide each staff member with the appropriate training, support, information and guidance to ensure that they are equipped to deliver high standards of care and achieve best practice.

The philosophy of the service is based on the Governments White Paper ‘Valuing People’ and we are committed to providing each service user with the appropriate support to enable them to exercise their rights as a human being, to be as independent as possible, and to have choice and control in their life and we will continue to work towards achieving social inclusion for all.
