Older Persons Service

Aims and Objectives

 

Overall Aim 

To provide people with learning disabilities with the opportunity to access a wide range of sessions within their own communities.

 

Objectives 

  • To receive referrals and plan appropriate services through the care planning process in accordance with FACS criteria

 

  • Respond swiftly and effectively to referrals

 

  • To provide a high quality, flexible, person centred service, which is monitored, reviewed and evaluated

 

  • Provide meaningful and educational support to allow individuals to develop their full potential

 

  • Provide a service based on equality to ensure that everyone is treated equally regardless of race, gender, ethnicity, age, sexuality and disabilities

 

  • Provide a safe environment for people who are elderly and/or present the onset of dementia

 

  • To support people in their own communities to maintain their identities and cultural needs

 

  • To support people to be part of their own communities and take part in everyday activities

 

  • Support people to access other recreational and educational opportunities

 

  • Support people to access mainstream services such as college courses and voluntary work

 

  • Provide people with a service that meets their assessed needs

 

  • To support people with their person centred plans

 

  • Work closely with service users, parents/carers and other professionals to make sure that the service is suitable

 

  • Work closely with parents/carers who are aged 65 and over to make sure that we provide support and information about carer’s assessments

 

  • Support people to be as independent as possible and not assume they need to have our service forever

 

  • Ensure that everyone participates fully in the planning of the groups

 

  • Ensure that everyone’s health, safety and well-being is a priority at all times

 

  • Provide experienced, trained staff at all times

 

Philosophy of Care 

The philosophy of the service is based on the Governments White Paper ‘Valuing People’ and we are committed to providing each service user with the appropriate support to enable them to exercise their rights as a human being, to be as independent as possible, and to have choice and control in their life and we will continue to work towards achieving social inclusion for all.
