What is a Support Plan?
A Support Plan shows what your needs are and what outcomes you would like to achieve. The details on the Support Plan will effect how your Personal or Individual Budget can be spent.
How do I plan my support?
The Plan needs to be written, but it can be a long or as short as necessary. It could also include picture to help you to explain what you need. You can write it yourself, or you could ask a friend, family member, your care manager or another advocate to help you.
How long will it take to write a Support Plan?
We will complete a Support Plan with you within fours weeks of an assessment being finished.
