Leaving care support

Leaving care support is provided by the 16+ team which is part of the Social Care and Education Department. The 16+ team provides a dedicated personal advisor role for the looked after young people and care leavers in the city.

What is the 16+ team?

The team works with young people who are entitled to support as outlined in the Children (Leaving Care) Act 2000. In April 2017 the Children and Social Work Act became law and the provision for care leavers has been extended to include young people aged 22 – 25 who are not in education where support is requested.

The team will work closely with you, your social worker, your carers and other professionals to help ensure that you are ready and able to make a successful move into adulthood.

How can we help?

We will support and advocate on your behalf, this will include carrying out a six monthly pathway review meeting with you and other agencies including Connexions, Health, Housing, Education and other organisations who maybe supporting you.

What happens when you are referred to the 16+ team?

Your personal advisor will get in touch with you around your 16th birthday to explain their role. They will remain involved with you whilst you are in care and after you leave care, they will attend your pathway meetings, along with other agencies and will take on some tasks for you but most importantly they will advocate on your behalf. Your personal advisor will take on responsibility to support you once your social worker officially ends their role with you.

Our aim is to make sure you are provided with the right opportunities to develop your skills and your learning to make a successful move into independence.

16+ team drop in service

We also offer a drop-in service for care leavers who are allocated to the service, to access support from the teams’ duty worker, each working day. 

Care leavers would be expected to contact their own personal advisor in the first instance and the duty worker is available for situations that cannot wait until their own worker is available.

Drop in service sessions

  • Monday to Thursday: 9am - 4.30pm
  • Friday: 9am - 4pm
