All application forms relating to the Licensing Act 2003 have been prescribed by central government. The forms are available via the links below. Once you have downloaded a form you can either fill it in on your computer, or print the form and fill it in by hand.
The Police ask all applicants who wish to hold a licence or be named as a designated premises supervisor to complete their form Transfer of Premises Licences and Variations of Designated Premises Supervisors. This should be sent to the Police with their copy of the application.
